The following represents the Agreement between Mary Lou Floyd, ML Mentor & Guide, hereinafter “Coach” and the Client, hereinafter “the Client”. The Services shall be for the Family Maintenance Program, as set forth below (“Services”).

SERVICES

The Coach will assist the Client through the Family Maintenance Program by providing open, honest feedback, asking questions, and guiding Client through assessments and/or worksheets.
The Services will be delivered by Zoom [or in person if the method is mutually agreed on] as follows:

  • Meet 3 times over 2 months
  • Sessions will be 60-90 minutes
  • Email and text support in between sessions (M-F, 9 AM to 5 PM)
  • Includes worksheets

To protect confidentiality, it is my practice to use WhatsApp for text communications. WhatsApp uses end-to-end encryption on all chat conversations. The Coach will provide you with the necessary information to use this application.

CANCELLATION

Please provide at least 24-hour notice if you need to cancel/reschedule. If you cancel/reschedule less than 24 hours before your appointment, you will need to email the Coach at [email protected] and send a text message to the Coach. Appointments will be scheduled through an online portal that does not allow for changes less than 24 hours before your appointment except through the Coach. This time slot was reserved for you and was not available to any other client. Preparation considerations were made for your session. No-shows will be counted as a session and not eligible to be rescheduled.

REFUND POLICY

I do not offer refunds on coaching services. I am here to coach you to your highest level of success, and part of that is requiring a commitment from you that you are fully invested in your coaching program. It is to YOUR benefit to be committed and 100% in, just as I will be for you.

CONFIDENTIALITY AND LIMIT OF LIABILITY

Any information divulged to the Coach by the Client shall remain completely confidential, except in rare circumstances where decreed by law, i.e. court issued subpoena, illegal activity, or admission by the Client of the intent to commit an illegal act or seriously harm someone. The Client acknowledges the obligation of the Coach to cooperate fully with law enforcement and other authorities as required by law.

The Client acknowledges that the Coach is not in the business of providing advice, medical, legal, or otherwise, and the Client should retain the services of a qualified professional in any particular area where such qualified advice is required. Furthermore, the Coach does not provide therapy or mental health counseling. Clients with psychological or other medical problems should consult a qualified healthcare professional. Any action taken in regard to therapy, medical advice, mental health counseling, or any other activity not directly related to the Services provided to the Client by the Coach will be at the sole discretion of and will remain the sole responsibility of the Client, and the Coach will not be held accountable and shall remain blameless for any consequences arising as a result of any professional therapy, counseling or any other activity not directly related to the Services provided by the Coach.

TERMINATION

Coaching under the terms of this agreement will continue for the number of sessions and months as described under “SERVICES”. Only under extenuating circumstances can this Agreement be postponed.

ENTIRE AGREEMENT

This Contract constitutes the entire agreement between the Client and the Coach. The Coach shall not be held liable and shall remain blameless for any adverse or other consequences, including but not limited to financial, personal, employment, or other losses, arising out of decisions or actions the Client may make as a result of the Services provided by the Coach.

FAMILY MAINTENANCE PROGRAM


The Family Maintenance Program is for clients who have completed the Family Strengthening Program,


What's Included:

  • All sessions are 1:1 and customized to your unique needs
  • Recommended duration is three (3) sessions over 2 months.

This follow-up program is designed to provide ongoing guidance and refinement of your Family Strengthening Plan.


All sessions will be held in secure member portal. Additional support materials will also be available in the library.
